Description:
Provides spiritual support services to patients and families reflective of patient/family’s desire/belief system and serves as consultant to Interdisciplinary Team for spiritual support issues and information.
Requirements:- Provides direct spiritual support and/or counsel to patients/families in keeping with their acceptance of this service and in accordance with their beliefs/desires and Hospice and professional standards.
- Counsels persons who are seeking religious or moral guidance.
- Assess spiritual strengths and needs on an ongoing basis.
- Maintain contact with and spiritual support for patient family/caregivers ? Participates in Interdisciplinary Teams, exploring and assessing potential spiritual needs of patients/families and documenting/reporting on services, activities, plans of care and patient/family contacts provided, in the form and timing compliant with Federal, State and Hospice requirements.
- Makes all reasonable efforts to maintain contact and involve clergy or appropriate representative of patients/families as appropriate per patient/family desire.
- Facilitates and supports staff, clergy and community groups to enhance sensitivity and dignity to spiritual concerns of patients/families experiencing terminal illness and grief.
- Facilitates family transition, as appropriate, into bereavement programs as part of pastoral care needs, including visits to patients/bereaved to provide spiritual comfort.
- Acts as resource to staff, providing support to staff/volunteers in coping with job-related stress, assisting in the development and implementation of training programs and assisting in QA/CQI efforts regarding spiritual care issues.
- Maintains personal professional competency.
- Participates in evening/weekend coverage responsibilities.
- Organizes and leads religious services and officiates at special ceremonies and funerals/memorial services as requested.
- Provides support within the appropriate faith tradition per patient/family preference by leading patients, patient family members and others in prayer, administering the sacraments, performing rituals, reading from sacred texts, etc.
- Maintains quality and productivity standards.
- Participates in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested.
- Participates in agency and community programs, including supervision, to enhance learning and performance and remains current in topics/research in hospice, spirituality and grief.
- Participates in the orientation of volunteers and staff and supervises chaplaincy field placement students when applicable
- Adheres to organizational policies including confidentiality regarding patients, families, staff, and the organization
- Takes primary responsibility within the IDT for communicating patient/family spiritual beliefs and how they influence and relate to the overall plan of care
- Completes other assignments and tasks as requested.
Typical Physical Demands/Working Conditions:
- Sit, stand, stoop, bend, reach, lift and walk, climb steps in patient homes, office settings and facilities
- Communicate by phone, writing speaking
- Lifting up to 20 pounds
- Drive short/long distances
- General fast-paced work environment
- Must be able to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens, smoke, no chairs
- Must be able to complete EMR at bedside in varied environments
Other:
- Driver’s license, personal automobile and current automobile liability insurance
- Frequent travel may be required
- Regular sitting and computer use required
Qualification:
- Master’s degree in Divinity from accredited seminary or school of theology, ordination by a religious or appropriate certification in hospital/ pastoral ministerial counseling preferred.
- Five years’ experience in pastoral care or counseling preferred. Hospice experience preferred.
Knowledge of the spiritual issues common during illness, death and bereavement.
- Skill in counseling, interviewing and communicating with patients, families and staff.
Intermediate proficiency in Microsoft Word software and ability to learn NDOC system required. Demonstrated ability to be open, sensitive, flexible and experienced in working with patients/families dealing with life-threatening illness and death.
- Skill in establishing and maintaining effective working relationships.
- Flexible and able to function in stressful situations.
- Good organizational skills, strong verbal, written, and presentation skills. Ability to manage multiple tasks in a busy office environment. Exceptional customer service skills.
- Ability to work with and maintain confidentiality of patient data. Must demonstrate basic computer skills
