Now Hiring: Primary Care Physician (MD) Geriatric Care | California

Location: Los Angeles, CA (On-site, Home Visits, and Virtual)
Schedule: Mon--Fri | 8 AM5 PM | Flexible + On-Call Rotation

Join our client's growing PACE program, an innovative care model delivering comprehensive medical services to high-risk seniors across Los Angeles. You'll be part of a collaborative team transforming how older adults experience healthcare.

Minimum Requirements:

• Doctorate degree in Medicine
• 2+ years experience as a Physician
• 1+ year in any of the following:
  
  • Geriatric Care
  • Senior Care
  • Long-Term Care (LTC)
• Board Certified in Family Medicine or Internal Medicine
• Active Licenses/Certifications (All Required):
  
  • California MD license
  • DEA & CDS Registration in CA
  • AHA Basic Life Support (BLS) Certification
• Valid California Drivers License , reliable personal transportation, clean driving record, and auto insurance

• Preferred:

• PACE experience (1+ year)
• Bilingual in Spanish

Key Responsibilities:

• Deliver holistic primary care to PACE participants, including exams and chronic disease management
• Manage your own patient panel in coordination with a complex care RN
• Initiate and review orders for medications, diagnostics, and referrals
• Provide care across PACE center, participant homes, SNFs, ALFs, and hospitals
• Collaborate with Interdisciplinary Team (IDT) including behavioral health, pharmacy, and rehab
• Participate in care planning, Service Determination Reviews , and clinical team meetings
• Document accurately in the EMR
• Participate in after-hours phone call rotations , supported by nurse triage

Schedule:

• Monday-Friday, 8 AM 5 PM
• Flexible daily schedule with occasional travel to care settings
• Rotating on-call (remote) coverage
