Summary:

We are seeking a compassionate and driven Physician to build and manage a dedicated panel of patients across independent and assisted living facilities. Essential Duties & Responsibilities:
  • Patient Panel Development : Build and maintain strong patient relationships by developing a dedicated panel and providing continuity of care.

  • Direct Patient Care : Conduct assessments, manage chronic conditions, and deliver preventive services as the primary physician for residents.

  • Routine Rounds : Perform regular visits at assigned facilities, addressing both ongoing and acute health needs.

  • Care Coordination : Partner with facility staff, families, and interdisciplinary teams to create personalized, holistic care plans.

  • Documentation & Compliance : Ensure accurate, timely EMR documentation while adhering to all regulatory standards.

  • Patient & Family Education : Provide education on preventive health, chronic condition management, and wellness strategies.

  • Quality Improvement : Participate in initiatives that drive better outcomes and elevate the standard of care.

Qualifications:

 

  • Medical Degree (MD or DO) from an accredited institution
  • Board Certification in Family Medicine, Internal Medicine, Geriatrics, or Hospital Medicine
  • Active, Unrestricted Medical License in the state of practice
  • DEA Certification and current BLS/ACLS certification
  • Proficiency in EMR systems and timely documentation practices
  • Strong Interpersonal and Communication Skills, with a commitment to patient-centered care

 

This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required. If so, you will be provided with thirty (30) days notice of this requirement. From that time forward, please be advised that Periodic motor vehicle (MVR) checks may be conducted as a condition of, and/or for, continued employment.
