Position Summary:
The Structural Heart Coordinator will utilize advanced clinical knowledge and skills, He/she will collaborate with physicians and midlevel’s to provide direct care and oversee care coordination for patients in the Structural Heart Program Prioritizing the patient experience and offering exceptional customer service are of upmost importance.
The coordinator is responsible for the planning, direction, and operation of the administrative and clinical aspects of the patient coordination component for the Structural Heart program. Manages and coordinates resources for the delivery of care to patients and their families. In collaboration with the physicians, while impacting patient care including but not limited to, screening, education, consultation, and coordination of care from the pre-procedure through procedural recovery and outpatient clinic. Manages clinical education plans from strategic concept development through implementation and execution.

ESSENTIAL FUNCTIONS:
Coordinates the daily operational activities required for effective functioning of the patient component of the Structural Heart Program. The Structural Heart Program consists of advanced treatments and procedures such as Transcatheter Aortic Valve Replacement (TAVR), Transcatheter), Valve in valve and Left Atrial Appendage Occlusion (WATCHMAN).

Acts as a liaison between the hospital, Structural Heart Program, and other aspects of the Heart and Vascular Institute. Performs all follow up support to patients and physicians. Works with the Medical Director and HVI leadership to develop, implement and track relevant quality outcome and patient satisfaction measures.

Collaborates as a skilled communicator, working among multiple departments simultaneously. Demonstrates ability to be highly organized with foresight for prioritization and action.

Ensures compliance and achievement of Hospital wide Quality Improvement Plan. Works with the Medical Director and CVI leadership to regularly update and identify appropriate quality improvement metrics. Oversees ongoing quality assurance activities by collecting, measuring, and analyzing statistical data, collects and reports outcomes (internal/external) including but not limited to length of stay, mortality/morbidity, complications along with operational and functional measures.

In collaboration with physicians, conducts assessment of patient needs by evaluating/interviewing all new /returning patients. Triages patients to appropriate care resource (includes direct contact and telephone encounters). Reviews patient history and other supporting clinical data to facilitate optimum care.

Organizes care conferences with the multi-disciplinary staff during the screening process and promotes a team approach to the patient program. Effectively explains and educates patients and/or family members about the procedures. Teaches based upon assessment, expressed need from patient and/or family or the initiation of new treatment.

Participates in the development of policies, procedures, and standards of patient care. Participates in developing and maintaining specific patient care plans, clinical pathways, and protocols.

Receive and review referrals for appropriateness, reviewing with an attending as needed.
Obtain all outside imaging, and other medical records. Coordinate initial work up and diagnostic testing for patients and families. Also assists families coming in from out of town in finding lodging and amenities. Perform assessments and initial work up. Have all testing available for review by cardiologist and CTS at valve clinic visit. Participate in development of treatment or management plan. Assists in clinic alongside mid-levels and specialists. Patient and family education. Coordinates specialty visits.Drag Edit Delete
Facilitate direct contact between specialist and referring physician regarding treatment plan.
Coordinate any additional testing i.e. cath, TEE, CT/MRI. Upload required data to appropriate vendor and maintain communication with device representatives.Minimum Qualifications:
Minimum Education
Graduate of an accredited school of nursing- BSN required

Minimum Work Experience
Background in cardiac preferred and strong skills in coordination, facilitation, patient communication and physician relationships.
